Minimally Invasive Therapies for Adenomyosis: Improving Women's Quality of Life While Minimizing Harm

The Strategies and Techniques for Minimally Invasive and Noninvasive Treatment of Adenomyosis forum, an integral part of The Fifth Yangtze International Summit of Minimally Invasive and Noninvasive Medicine & 2023 Belt and Road Innovation Forum on Minimally Invasive and Noninvasive Medicine, was successfully convened on July 9th.

Co-chaired by a group of esteemed medical professionals, including Oliver Cruz, President of the Mexican Association of Gynecological Endoscopy and Microsurgery, and Lin Hanmei, Director of the Department of Gynecology at the First Affiliated Hospital of Guangxi University of Traditional Chinese Medicine, the forum facilitated insightful lectures and discussions by six experts on minimally invasive therapies for adenomyosis. The primary goal was to identify strategies that effectively treat the condition, improve the patient's quality of life, and minimize harm.

Sevellaraja Supermaniam, an obstetrician and gynecologist from Malaysia, explored how High-Intensity Focused Ultrasound (HIFU) can assist patients with infertility caused by adenomyosis, noting that the therapy causes minimal uterine harm and plays a pivotal role in fertility protection.

Fu Chun, Director of the Department of Gynecology at Xiangya Second Hospital, Central South University, emphasized the progressive nature of adenomyosis and how HIFU provides a new and safe therapy to prevent uterine surgery or hysterectomy. He stressed the necessity for personalized, comprehensive management post-treatment.

In a presentation on the clinical effectiveness of combining focused ultrasound ablation with thermal balloon endometrial ablation, Pu Yuanfang, Chief Physician at People's Hospital of Nanchuan District, Chongqing, encouraged continuous exploration of options that preserve the uterus, alleviate symptoms, improve quality of life, and reduce recurrence for adenomyosis patients.

Peng Yan, Director and Chief Physician of the HIFU Center at Shenzhen Maternal and Child Health Hospital, discussed the adoption of comprehensive HIFU therapy after the failure of conservative treatment for adenomyosis to control clinical symptoms effectively. She suggested that repeatable treatments could reduce hysterectomy rates in case of recurrence.

Addressing the needs of patients seeking fertility, Liang Dong, Deputy Director and Chief Physician of the Department of Internal Gynecology at Foshan First People's Hospital, emphasized the importance of precision and personalization in treatment. He advocated for the judicious use of focused ultrasound ablation pre-treatment for adenomyosis patients wishing to conceive.

Wrapping up the forum, Guo Tao, Chief Physician at West China Second Hospital of Sichuan University, shared an insightful analysis on the role of HIFU in treating endometriosis and adenomyosis. While he acknowledged the numerous advantages of this approach, he also shed light on its limitations such as slow post-treatment lesion absorption, a high degree of dependency on a physician's experience for determining the appropriate treatment dosage and intensity, and potential risks associated with excessive energy. These remarks underscore the unique demands placed on doctors, emphasizing the complexity of HIFU treatment and the constant need for continued growth and development within this medical domain.
