Pioneering Progress in Gynecological Health: Insights from the Chongqing Summit

The sub-forum on Minimally Invasive and Noninvasive Treatment for Gynecological Diseases took place successfully on July 9. This event was part of The Fifth Yangtze International Summit of Minimally Invasive and Noninvasive Medicine & the 2023 Belt and Road Innovation Forum.

The sub-forum was co-chaired by Mustafa M.M. Aldam, Founder and President of the International Association of Cosmetic Gynecology, Huang Yan, Director of the Gynecology Department at Qiannan People's Hospital, and Li Meifang, Head of the Breast Disease Diagnosis and Treatment Center at the First Affiliated Hospital of Jiangxi Medical College. The sub-forum was a resounding success. Six experts in the field of "minimally invasive and noninvasive treatment for gynecological diseases" shared their insightful lectures, discussing the minimally invasive and noninvasive treatment of gynecological diseases such as early-stage cervical cancer, mammary fibroadenoma, and pelvic adhesion, among others.

Yang Linqing, Chief Physician at the Affiliated Hospital of Jining Medical College, presented a lecture on the "Robotic Laparoscopic Strategy for the Management of Deep Endometriosis." Yang explained that Deep Infiltrating Endometriosis (DIE) is a condition that requires long-term management. Effective treatment for this condition involves careful preoperative, intraoperative, and postoperative management and benefits from multi-disciplinary team (MDT) cooperation. Consequently, robotic management of DIE presents distinct advantages and is a recommended procedure.

Yang Hongwei, Chief Physician of Breast and Thyroid Surgery at Suining Central Hospital, presented a special lecture titled, "The Effectiveness and Safety of HIFU in Treating Breast Fibroadenoma." During his presentation, he highlighted the numerous benefits of using High-Intensity Focused Ultrasound (HIFU) for treating breast fibroadenoma. The key advantages of this method include its noninvasive nature, meaning it doesn't cause any physical trauma, risk of bleeding or infection, or leave any incision scars on the skin. Another major benefit is its capability for real-time monitoring and precise, conformal ablation. Moreover, the procedure can be performed under local anesthesia, thus significantly reducing the treatment time. Importantly, HIFU enables the treatment of multiple lesions in a single session and allows for repeatable treatments, making it an especially suitable option for patients with multiple or recurrent breast fibroadenomas. Lastly, the absence of radiation and chemical damage ensures the overall safety of this treatment method.

Dr. Zhu Lin, Chief Physician at the Second Hospital of Shandong University, delivered a lecture titled, "Tumor-free Operation and Parametrial Management in Laparoscopic Surgery for Early-Stage Cervical Cancer." He explained that Laparoscopic Radical Hysterectomy (LRH) can be conducted without a uterus manipulator, provided there is a comprehensive preoperative evaluation that includes staging, imaging, and self-assessment. Dr. Lin underscored the importance of understanding the treatment principles as set out by FIGO (International Federation of Gynecology and Obstetrics) staging and NCCN (National Comprehensive Cancer Network) guidelines. He stressed the necessity for physicians to comprehend the various hysterectomy QM subtypes, their respective applications, and to carefully select cases of C1 and extensive cervical types. He encouraged physicians to gain proficiency over time but warned against overstepping their expertise level, always prioritizing the patient's interests first.

Xia Tiansong, from Jiangsu Provincial People's Hospital, shared a lecture on "Focused Ultrasound Ablation Surgery for the Treatment of Breast Tumors," exploring the treatment of early breast cancer, locally advanced breast cancer, and breast fibroids from three perspectives: effectiveness, safety, and maneuverability.

Xiao Ziwen, Chief Physician in the Department of Obstetrics and Gynecology at the Affiliated Hospital of Guizhou Medical University, delivered a lecture on "Exploration of Surgical Methods for Stress Incontinence." He shared his experience with laparoscopic separation of pelvic adhesions and addressed several questions regarding surgical techniques.

Zhang Cai--Comparison of cosmetic outcomes and cost-effectiveness among different treatments for breast fibroadenoma (Photo/ISMINIM 2023)

Zhang Cai, Head of the Medical Research Department at the National Engineering Research Center of Ultrasound Medicine, presented a "Comparison of Cosmetic Outcomes and Cost-Effectiveness Among Different Treatments for Breast Fibroadenoma." She concluded that compared to other treatments, HIFU treatment for breast fibroadenoma is cost-effective and offers higher utility. It can treat more lesions in a single session, achieve a higher rate of complete treatment of multiple lesions, require shorter treatment time for individual lesions, and present no postoperative adverse events. According to her findings, patients who received HIFU treatment experienced improved physical and psychological conditions postoperatively.

The sub-forum "Minimally Invasive and Noninvasive Treatment for Gynecological Diseases," part of The Fifth Yangtze International Summit and 2023 "Belt and Road" Innovation Forum, was a significant success. Leading experts shared valuable insights on advanced treatments for gynecological diseases, including robotic laparoscopic strategies for endometriosis, the use of High-Intensity Focused Ultrasound (HIFU) for breast fibroadenoma, and laparoscopic methods for early-stage cervical cancer. These discussions showcased the benefits of modern noninvasive treatments, emphasizing their efficiency and cost-effectiveness, thereby enhancing collaborative patient care approaches in gynecology.
