Experts Share Views on Liver Cancer Treatment

On July 9th, treatment strategies for patients with liver tumors, one section of the 5th Yangtze International Summit of Minimally Invasive and Noninvasive Medicine & 2023 Belt and Road Innovation Forum on Minimally Invasive and Noninvasive Medicine was successfully held. 

This section was co-chaired by Professor Mohamed Hamed of Cairo University Affiliated Hospital, Egypt, Xu Ning, Director of the Oncology Department of Fenyang Hospital, Shanxi Province, and Li Yongjie, Director of the Second Ward of the Oncology Department of Hezhou People's Hospital. Five experts gave brilliant lectures on the treatment strategies for liver cancer, discussed the cutting-edge treatment options for liver cancer, how to choose between different treatment options for liver cancer patients in different conditions, and what to do with cases unable to receive surgery.

Yang Tong, Chief Physician of Ningbo Second Hospital, shared a lecture on the expectation of immunology treatment for solid tumors treated by HIFU. HIFU could solve two problems in treating solid tumors: complex tumor driver genes and diverse clonal origins. Specific antigens are difficult to find, and 1-2 specific CAR-T or DC cells do not solve the problem. The fortress problem is TME(tumor micro-environment). It is expected that HIFU could bring about a revolutionary change to Abscopal. HIFU is a new choice for patients with malignant solid tumors.

Professor Mohamed Hamed from Cairo University Hospital, Egypt, discussed HIFU in liver tumors; where do we stand? - Egyptian experience. He introduced the condition of HIFU treatment for liver cancer, which is not less effective than surgery while reducing the patients’ pain. Even patients who lose the chance of surgery can be treated by HIFU.

Feng Gang, Deputy Chief Physician of Mianyang Central Hospital, shares his views on HIFU in the application of hepatic malignancies. HIFU’s application in treating liver cancer includes primary liver cancer, intrahepatic cholangiocarcinoma, and liver metastases. Meanwhile, HIFU has contributed to the disciplinary construction since it is a unique non-invasive therapy, does not affect the progress of other treatments (chemotherapy, targeting, immunity), and improves the curative effect of medical oncology treatment as a whole.

Franco Orsi, Professor of the European Institute of Oncology, Italy, discussed percutaneous ablation for CRLM: technique and outcome. He gave a brilliant explanation of the minimally invasive tumor treatment and shared cases of percutaneous ablation for CRLM. Survival was comparable in the “ablation + partial hepatectomy group” to the “partial hepatectomy group,” potentially leading to long-term disease control with a lower complication rate. Thus ablation is favored over chemotherapy alone. At the same time, combining thermal ablation with chemotherapy reduces morbidity, mortality, and costs and improves survival.

Wu Mingsong, Director and Chief Physician Department of Oncology and Hematology, Beiliu People's Hospital, discussed clinical practice in the minimally invasive and non-invasive treatment of hepatobiliary and pancreatic cancer guided by integrated medical sciences. Initial progress has been made in the integrated medical sciences of hepatobiliary and pancreatic cancer, which deserves further exploration. Integrated medical sciences is an emerging medical system emphasizing “disease” to “patient,” embodying humanistic medicine. Minimally invasive intervention plus HIFU non-invasive ablation and organic integration of other systemic treatments and conditioning can benefit patients more.
